HealtheLife / Mammoth Patient Portal Proxy Authorization

Medical Record

Subject to the following limitations, access to a patient’s Mammoth Hospital Patient Portal account by a third person can be requested by submitting the Proxy Authorization Form. 

If you would like to grant access to your medical records to another adult, or if you share an email address with someone. Please complete the form below. We will send an invitation to access your medical records to the proxy’s email address you provide.


Minors:

Patients Aged 0-11 Years

  • Until the patient reaches 12 years of age, Parents/legal guardians may obtain access to the minor’s portal account with the parent’s email address.
  • Parent/legal guardian access automatically terminates when patient reaches 12 years of age to comply with California law*. Upon the patient reaching 12 years of age, the parent/legal guardian will receive an email notifying them of such access termination.

Patients Age: 12 Years

  • Patients are automatically disenrolled from the portal on their 12th birthday and will not be eligible to create a new portal account until they turn 13.
  • Patients, Parents, or Legal Guardians who need access to records during this time may contact Medical Records to complete an authorization form for copies, in accordance with privacy laws.

Patients Aged 13-17 Years

  • Minor patients in this age range may access the portal by creating an account with their own email address (cannot be the parent’s email address or one that parent has any access to).
  • To comply with California Law*, parent/legal guardian will not have access to the portal of a minor patient in this age range. If parent would like to obtain a copy of the portions of the child’s medical record to which they have the legal right to access, please contact the Mammoth Hospital Medical Records Department at 760.924.4041 or email HIMS.Email@mammothhospital.com.

Adults:

Patients Aged 18 Years and Older

  • All adult patients may access their health records online by creating a HealtheLife/Mammoth Patient Portal account with a unique and valid email address.

Proxy Access

  • Any adult patient may grant proxy access to his or her portal account to another individual by completing the Proxy Authorization Form.  
  • Patients with a shared email address must fill out a Proxy Authorization Form.  The first patient will have an invitation sent using the same process as an adult with their own unique email address. The second patient will have an invitation sent using the Authorized Representative fields.
  • The patient may terminate Proxy Access at any time by contacting the Mammoth Hospital Medical Records Department at 760.924.4041 or email  HIMS.Email@mammothhospital.com .

*Parent/legal guardian access to a minor patient’s health information is restricted due to California laws protecting minor confidentiality for certain types of treatment.
